What Is VA Aid & Attendance?

Aid & Attendance (A&A) is an enhanced VA pension benefit for veterans and surviving spouses who need help with daily activities or are housebound. It's designed to help cover the cost of in-home care, assisted living, memory care, or nursing home care.

Unlike basic VA healthcare, Aid & Attendance is a monthly cash benefit. You can use it to pay for:

Eligibility Requirements

To qualify for VA Aid & Attendance, the veteran must meet all three criteria:

1. Military Service Requirements

  • Served at least 90 days of active duty
  • At least one day of service during a wartime period:
    • World War II (Dec 7, 1941 – Dec 31, 1946)
    • Korean War (June 27, 1950 – Jan 31, 1955)
    • Vietnam War (Feb 28, 1961 – May 7, 1975 for those in Vietnam; Aug 5, 1964 – May 7, 1975 for others)
    • Gulf War (Aug 2, 1990 – ongoing)
  • Discharged under conditions other than dishonorable

2. Medical Requirements (Need for Aid)

Must meet at least ONE of these conditions:

  • Need help with at least two activities of daily living (bathing, dressing, eating, toileting, transferring)
  • Be bedridden
  • Be a patient in a nursing home due to mental or physical incapacity
  • Have limited eyesight (5/200 or less in both eyes, or concentric contraction of visual field to 5 degrees or less)
  • Have a mental or physical condition requiring care or assistance for protection from daily hazards

3. Financial Requirements

  • Net worth limit: Total assets (excluding home and vehicle) under ~$155,356 (2025 limit, adjusted annually)
  • Income limit: Countable income minus unreimbursed medical expenses must be below pension limit
  • Medical expenses count: Assisted living costs, caregiver costs, and medical expenses reduce countable income

Understanding the Income Calculation

The VA uses a formula called IVAP (Income for VA Purposes). It subtracts unreimbursed medical expenses from your income. This is crucial because assisted living costs count as medical expenses, often reducing countable income to zero or near-zero—making many veterans eligible.

Example: A veteran receives $2,500/month in Social Security. Their assisted living costs $5,000/month. IVAP = $2,500 - $5,000 = $0, likely qualifying them for maximum benefits.

How to Apply for Aid & Attendance

Step 1: Gather Documentation

You'll need:

  • DD-214 (discharge papers): Proves military service. Request from National Personnel Records Center if needed.
  • Medical evidence: Doctor's statement of need for assistance, diagnoses, current medications
  • Financial records: Bank statements, investment accounts, real estate, income sources
  • Proof of medical expenses: Assisted living contract, caregiver receipts, pharmacy costs
  • Marriage certificate: If applying with or as a surviving spouse
  • Death certificate: If surviving spouse is applying
